The setup cost is only the time it takes to organize Azure resources so you can operate effectively and figure out how to manage different environments (dev/test/sit/uat/prod, etc.). Also, how to enable multiple developers to work on a single data factory without losing changes or conflicting other changes.

I don't see a cost; it appears to be included in general support. I have been told that you have to be very careful because it can blow out. I have not experienced it yet, but I've been warned that as Azure ingestion increases, the costs can rise. In my opinion, the price is competitive.

It depends what the threshold is. I see we're not at that threshold right now, so it's pretty decent right now.

In terms of licensing costs, we pay somewhere around S14,000 USD per month. There are some additional costs. For example, we would have to subscribe to some additional computing and for elasticity, but they are minimal. For disaster recovery and readability setups, we did that on Data Lake.
